A great team of Trusted Assessors and Community Care Assistants at Essex Cares Limited have helped bring a community reablement mindset and capacity into local hospitals.
To help get people up and moving, a ward-led enablement service has been mobilised at Broomfield Hospital (Braxted Ward), Basildon Hospital (William Harvey Ward) and Southend Hospital (Blenheim Ward).
Patients are supported to regain confidence and independence with personal care tasks, so that they are able to do these things for themselves when they return home. Aside from helping them to be as independent as possible during their hospital stay, the team reduce the likelihood of extended stays and minimise the need for ongoing home care once discharged.
Their presence in the hospital has been welcomed, while the team were supporting one patient to mobilise, a neighbouring patient (who had refused all efforts from the hospital to engage previously) called out “Get me my frame, I’m coming with you. We’re doing it together!”
